Requesting Your Pet’s Records
To request your pet’s medical records, please use one of our convenient contact methods below:
- Otto App or Website Widget: Submit your request directly through the Otto app or the widget on our website.
- Email: Send an email to hsnyrecords@gmail.com.
- Text: Text us at 212-752-4840.
Make sure to include the following details with your request:
- The first and last name of the owner on the account
- The phone number associated with the account
- The pet’s name
Sending Records to the HSNY
Send your pet’s records to us by emailing hsnyhospital@gmail.com.
Please include this information with each record:
- The first and last name of the owner on the account
- A phone number where we can contact you
- The pet’s name
Test Results
Please note the approximate timeline for your pet’s results:
- Blood tests and & urinalysis: 7 to 10 days
- Stone analysis: 4 weeks
- Rabies titers: 4 to 6 weeks
- Fecal test results: We will call you if your pet’s results are positive. If you would like to follow-up, or request a copy of your results, please follow the instructions below.
If the wait time has been longer than anticipated, please send an email HSNYRecords@gmail.com. Make sure to include the following information:

- The phone number associated with the account
- The pet’s name
Pharmacy & Medication Requests
To request a refill for your pet’s medication or prescription diet, please use one of our convenient methods:
- Otto App or Website Widget: Submit your request directly through the Otto app or the widget on our website.
- Text: Text us at 212-752-4840.
Please include the following details with your request:
- The first and last name of the owner on the account
- The phone number associated with the account
- The pet’s name
- The name and quantity of the medication you are requesting
- Specify if you prefer a written prescription, in-person pickup, or online fulfillment (Chewy, PetMeds)
⚠️ Important: All requests are for existing patients only and must be reviewed and approved by your pet’s veterinarian. Please submit your request at least one week in advance to ensure your pet’s medication is ready on time.
